New Patients

Welcome to Merryman Family Dentistry! We are currently accepting new patients and look forward to hearing from you. To make an appointment with us, please call our office at your convenience. Our New Patient paperwork and Notice of Privacy Practices Acknowledgement forms are on the Documents page. These forms can be printed and completed to take with you to your first appointment. You may also choose to arrive 15 minutes early and fill them out at the office. If you would like to read our HIPAA Notice of Privacy Practices, we have also made that available in our links. Our office has non-metered street parking. Our office is fully handicap accessible. If you have any specific needs, please mention them to reception when you call.

If you are coming from a previous dental office, your dental records can be transferred to us. This will help ensure the best possible care provided to you in our office. The Records Release form is also on the Documents page, which you can print, complete, and send to your prior dental office so that your records can be obtained by us.

Insurance

We are a fee for service, non-participating office. What this means for our patients is that we ask for payment in full at time of services, claims are sent by us electronically the same day for any patients with out-of-network dental benefits, and the insurance reimbursement payment is sent directly to the patient. If you are unsure whether your dental insurance pays for out-of-network services, you can call the number on the back of your card for policy details. Due to our decision to be a non-participating provider, we are happy to discuss payment agreements with patient should the need arise. Please speak with reception if you are in any need of financial accommodations.

We are also incorporating CareCredit to accommodate payment for larger payment of services.
